Your road to successful speech begins with an evaluation. A therapist will
evaluate and identify your communication needs. The evaluator then designs an
appropriate treatment program to help you meet your communication goals.
Scheduling an evaluation is easy.
Click on, complete and submit the
Client History Information form. We will contact you after we receive
this form to schedule an appointment. You may be asked to fill out other forms
after we receive your initial scheduling request. We will let you know which
forms to complete. The New Client Handbook provides helpful information about
our policies and procedures.
